Procalcitonin

Low Vitamin D is associated with high inflammatory markers

Makes Sense Vitamins are antioxidants. If you don't have enough vitamins you have more inflammatory markers.

There are numerous studies between vitamin D levels and inflammatory markers
In the ICU low vitamin d is associated with greater severity in a number of conditions including ventilator associated pneumonia.

In one study with Vitamin D and pneumonia giving Vitamin D decreased the levels of procalcitonin The levels of PCT were significantly decreased (p=0.001) in the treatment group (– 0.02 ± 0.59 ng/mL) compared to that of placebo group (0.68 ± 1.03 ng/mL)

Pathophysiology
Procalcitonin is a peptide precursor of calcitonin
Calcitonin is used in calcium homeostasis.
Calcitonin is secreted in the C-cells of the thyroid gland after proteolytic cleavage of procalcitonin.
The thyroid C cells produce Procalcitonin mRNA which produces procalcitonin producing calcitonin
PCt should be low <0.05.

Inflammation causes production of CT-mRNA directly to procalcitonin bypassing conversion to calcitonin.

Normally procalcitonin only from thyroid
In inflammation procalcitonin can be produced from many cells in the body
Procalcitonin is an inflammatory marker which is induced by pro-inflammatory cytokines Furthermore, vitamin D can result in a reduction of the pro-inflammatory cytokines and thus, is supposed to indirectly reduce procalcitonin levels in serum
In conclusion, our results indicate that vitamin D supplementation can significantly reduce the procalcitonin in (VAP) patients, and must be considered as a preventive and/or therapeutic strategy.

Procalcitonin more specific than ESR and CRP for bacterial illnesses
If procalcitonin high more likely bacteria procalcitonin low more likely viral
Can be elevated in autoimmune conditions - kawasaki disease as opposed to not elevated in rheumatoid arthritis lupus

38F with PMH anxiety, depression, cholecystectomy, RnY gastric bypass 2012, and asthma presented with LE weakness and severe depression. Work up positive for slightly positive urinalysis, low vitamin D, elevated procalcitonin. Will keep in hospital for IV antibiotics and severe depression.

Vitamin D 6.1, procalcitonin 8.7, lactate 3.03

Procalc elevated secondary to sepsis + low vitamin d?
